Scar Reconstructions
It is important one to know that the scars will never disappear and that there is no surgical technique that leaves no scar. Whether a surgical scar would be visible or not, can someone prevent it if he has in mind the following:
Scars that are vertical to the body lines in an area will be visible.
Scars in some areas of the body develop hypertrophic scars, for example the shoulder and the sternum area, while some leave small or non-visible scars, for example the eyelid and the ear area. The big pressure when the surgeon makes an incision will leave a wide scar. The younger the patient is, the more obvious scar will be left. The more relaxed a skin is, the more cosmetical a scar is created. The patient’s background must be taken under consideration. The plastic surgeon must always make the incisions in areas that can “hide” the scars, like behind the ears, in the nose, in the scalp, etc.
Hypertrophical and often adhesive scars from traumas or burns should be treated based on the above principles. The scar removal leaves often an important skin deficit; something which should have been anticipated, so as the placement of a tissue dilator must proceed to gain some skin. The skin dilator is a silicone balloon, which is placed under the skin and gradually dilates with natural serum fill in 2-3 months’ time. Its removal leaves enough skin excess, to cover the deficit that the scar removal leaves. The modification of the scar, to place it in parallel with the body lines, using the known Z-plasty techniques, may increase the scar’s length, but would render it indiscernible.
